Liability Waiver
Welcome to Studio Sol! Please be aware that our schedule may change without notice. All purchases for services, including classes, are not refundable unless canceled at least 4 hours in advance. We kindly ask that you arrive at least ten minutes prior to class, as we start on time. Doors lock at the beginning of class but valuables should be left in your car or at home. Please remember to take your belongings home with you, as we are not responsible for their safekeeping. By booking a class, you agree to our terms below, including the possibility of being photographed during sessions solely for promotional content. If you have any health concerns or injuries, consult with your doctor prior to engaging in any classes. Please inform the instructor if you have any concerns and be mindful to modify for your body and take breaks as needed.
I acknowledge and understand that participating in yoga and pilates classes involves significant physical exertion and carries inherent risks that may lead to injury or even death. By choosing to engage in these classes, I willingly accept and assume all associated risks. I recognize that injuries are a common occurrence in these settings and voluntarily elect to participate despite this understanding. Furthermore, I accept full responsibility for any risks that may arise from my involvement in these classes and agree to release, indemnify, and hold harmless Studio Sol LLC from any liability, claims, or legal actions related to any loss, damage, or injury, including death, that I may sustain while participating. This includes, but is not limited to, injuries resulting from negligence or any improper conduct by Studio Sol.Hershey LLC. I am aware that the classes involve potentially hazardous movements and exercises(including using weights, foam blocks, yoga mats and straps), which may lead to various risks such as injuries from exercise equipment, slips and falls on slippery surfaces, or strains and tears in muscles. I confirm that I possess the necessary mobility and fitness level to safely engage in these classes and that, to my knowledge, I do not have any physical, medical conditions that could jeopardize my safety or the safety of others during participation.
I represent and warrant that I am in good physical health and do not suffer from any medical condition which would limit my participation in the classes offered at Studio Sol , LLC. I understand it is my responsibility to inform the instructor of my limitations before class begins and/or allergies to oils(lavender) used on chill towels. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any of the classes, programs, or workshops and using additionally using weights or straps . I understand the risks associated with the activities offered by Studio Sol Hershey, LLC and I agree to follow all instructions so that I may safely participate in classes, workshops, or other activities.
I hereby WAIVE AND RELEASE Studio Sol , LLC, its owners, officers, employees, and and instructors from any claim, demand, cause of action of any kind resulting from or related to my participation in the programs offered at the facility. In taking part in the exercise classes, workshops, or other activities at Studio Sol Hershey, LLC, I understand and acknowledge that I am fully responsible for any and all risks, injuries, or damages, known or unknown, which might occur as a result of my participation in the classes, workshops, or other activities.
I agree that I am over 18 years of age. If less than 18 years of age, I hereby agree to have a parent or guardian read and sign off on this waiver and gain their approval for participation at SOL
If I am signing as a parent or caregiver of a minor, I hereby, give my consent to the statements of this waiver for the minor that I represent or that is under my guidance or care. I will assume all responsibilities and accountability for my minor's actions at Sol.
I have read the above release and waiver of liability and fully understand its content. I am legally competent to sign and voluntarily agree to the terms and conditions stated above. The laws of the state of Pennsylvania apply to this waiver of liability and any causes of action hereunder.
